Multiple Worksite - File Layout and Data Mapping

The default file name for the Multiple Worksite report is <three-character Site ID>MWR.txt.

The following table shows the positions, field names, descriptions, data source or valid values, and whether the field is required for the Multiple Worksite report.

Quarterly Wages

The total wages (taxable and non-taxable) paid to employees during the entire reference quarter are subject to Unemployment Insurance taxes. The wages for all worksites should match the wages reported on the Quarterly Contribution Report.

The valid value is the sum of UI wages for all worksite employees for the quarter.

The UI wages value is retrieved from the UI Wages field on the Employee Information form accessed from the NY Quarterly Wage Report page.

Comments

Explain any significant changes in employment or wages due to store closure, layoffs, bonuses, seasonal changes, etc. If any units of your firm are being reported for the first time following the expansion of operations or purchase of units from another firm, describe the business activity(s) conducted at each establishment. This will assist BLS in assigning industrial classification codes to the new unit(s). In addition, if units were purchased from another firm, provide the firm's name, the transaction's effective date, and the seller's UI number, if known. If units have been sold to another firm, please provide the name of the firm, the effective date of the transaction, and the UI number of the purchaser, if known.
