Quarterly Wage and Multiple Worksite Report - Setup - Worksite Information

Use the VT Quarterly Wage Report - Setup - Multiple Worksite page to define each physical worksite location for the employer and map one or more location codes to it. During the data load, the system reads these worksite records and uses the mapped location codes to group employees by worksite. Each employee is assigned to a worksite by matching their location code against the codes listed in the Locations field of the matching worksite record. Wage totals are then aggregated per worksite for the MWR submission file and verification reports.

Perform the following setup procedure before creating the Quarterly Wage and Multiple Worksite report.

  1. On the VT Quarterly Wage Report page, select Setup, then Worksite Information.

  2. On the VT Quarterly Wage Report - Setup - Multiple Worksite page, filter records using the filter criteria. To enter filter criteria:

    1. Select an option in the first field.

    2. Select or enter a value.

    3. Select Add if the button is enabled.
      Refer to Search for records for more information.

      • To add a record, select Add Record. Refer to the Field descriptions and enter valid information in the required fields. Select Save, and then Yes.

      • To edit a record, select the ellipsis in the Actions column and then select Edit. Update the information and select Save.

      • To delete a record, select the ellipsis in the Actions column, select Delete, and then select Yes.

Field descriptions

The following table describes the fields on the Worksite Information form of the VT Quarterly Wage Report - Setup - Multiple Worksite page.

Field

Description

Worksite Code

A unique identifier for the worksite.

Used during report generation to group employees by physical work location.

This field is view-only and cannot be updated after saving the record.

The limit is 21 characters.

Reporting Unit Number

The five-digit numeric reporting unit number assigned by the Bureau of Labor Statistics (BLS) to identify this worksite in the Multiple Worksite Report (MWR) electronic file.

Legal Name

The legal business name of the worksite location as registered with the state.

The limit is 35 characters.

Trade Name

The operating or trade name of the worksite, if different from the legal name.

The limit is 35 characters.

Street Address

The street address of the worksite location.

The limit is 35 characters.

City

The city in which the worksite is located.

The limit is 30 characters.

State

The two-letter state abbreviation for the worksite location.

Zip

The five-digit ZIP code for the worksite.

Zip Ext

The four-digit ZIP+4 extension for the worksite address.

Worksite Description

A descriptive label for the worksite (for example, the building name or department name).

The limit is 35 characters.

Primary Comment Code

An optional two-character BLS-defined code that explains a significant change in employment or wages at this worksite (for example, a strike or ownership change).

Secondary Comment Code

An optional second two-character BLS comment code, used when more than one change event applies to the worksite in the reporting quarter.

Third Comment Code

An optional third two-character BLS comment code, used when a third change event applies to the worksite in the reporting quarter.

Comments

Additional comments about the worksite or changes reported for the quarter.

The limit is 50 characters.

Included Locations

A multi-value list of location codes whose employees are assigned to this worksite. During report generation, the system matches each employee's location code against these values to determine which worksite record to use to write the employee's wages.

For example, if the Worksite Location selected on the VT New Hire Report - Setup - Employer page is Base Location, refer to the Location Table page and find the Location Code associated with the base location. Enter the location code for the base location in this field and select Add.

You can associate multiple locations with a worksite code.
