Use the VT Quarterly Wage Report - Setup - Transmitter page to store information about the organization that electronically submits the quarterly wage file to the Vermont Department of Labor (VDOL). The transmitter is typically the employing district itself, but it may also be a third-party payroll service bureau submitting on the employer's behalf.
Perform the following setup procedure before creating the Quarterly Wage and Multiple Worksite report.

On the VT Quarterly Wage Report page, select Setup, then Transmitter.

Refer to the Field descriptions and enter or select valid information.

Select Save, then Yes.
Before saving, you can use the Reset button to discard any unsaved changes and restore the fields to their last saved values.

The transmitter organization's nine-digit federal employer identification number (FEIN). |

The name of the transmitter organization. The limit is 50 characters. |

The city portion of the transmitter's mailing address. The limit is 25 characters. |

State |
The two-letter state abbreviation for the transmitter's mailing address. |

Zip |
The five-digit ZIP code for the transmitter's mailing address. |

Zip Ext |
The four-digit ZIP+4 extension for the transmitter's mailing address. |

Contact Title |
The title of the person at the transmitter organization responsible for the filing (for example, Payroll Manager). The limit is 30 characters. |

Contact Person Name |
The full name of the contact person at the transmitter organization. The limit is 30 characters. |

Contact Phone |
The ten-digit phone number for the transmitter contact person. |

Phone Ext |
The four-digit phone extension for the transmitter contact person. |

Authorization Code |
A six-digit numeric code assigned by the Vermont Department of Labor to authorize the transmitter to file on behalf of an employer. |

POA Number |
A three-digit numeric Power of Attorney number assigned by VDOL for third-party reporting agents filing on behalf of an employer. |
