Staff Injury Report - Maintain Data

Use the IN Staff Injury Report page to add, update, delete, search, and print staff injury incident records for Indiana state reporting. This page allows users to manually enter and maintain individual staff injury incidents, including the date of injury, whether it was reported to the worker's compensation carrier, whether the employee missed workdays, and whether it was reported publicly. The data is used to generate a hardcopy verification report (PDF or Excel) and an electronic survey file for submission to the state.

If errors are encountered during the Load data process, you can correct the source data on the Staff Information form of the IN Staff Injury Report page. After corrections, you must purge the data.

PowerSchool recommends that you correct the report’s source data that is retrieved from the Core application.

  1. On the IN Staff Injury Report page, filter records using the filter criteria. To enter filter criteria:

    1. Select an option in the first field.

    2. Select or enter a value.

    3. Select Add if the button is enabled.
      Refer to Search for records for more information.

      • To add a record, select Add Record. Refer to the Field descriptions and enter valid information in the required fields. Select Save, and then Yes.

      • To edit a record, select the ellipsis in the Actions column and then select Edit. Update the information and select Save.

      • To delete a record, select the ellipsis in the Actions column, select Delete, and then select Yes.

      • To download the information, select Download, then select Download PDF or Download as an Excel Document. The default file names are:

        • Excel: IN Staff Injury Report_<MMDDYYYY_hhmmss>.xlsx

        • PDF: IN Staff Injury Report_<MMDDYYYY_hhmmss>.pdf

Employee Number

The employee’s identification (ID) number.

Select the Lookup icon to search for and select an employee number.

This field is view-only and cannot be updated after saving the record.

The field value is retrieved from the Employee Number field on the Employee Information page.

Last Name

The last name of the employee.

This field is view-only.

The field value is retrieved from the Last Name field on the Employee Information page.

First Name

The first name of the employee.

This field is view-only.

The field value is retrieved from the First Name field on the Employee Information page.

Middle Initial

The employee’s middle initial.

This field is view-only.

The field value is retrieved from the Middle Name field on the Employee Information page.

Injury Date

The date the staff injury occurred.

This field is view-only and cannot be updated after saving the record.

The field value is retrieved from the INJURY DATE field on the STAFF INJURY situation type of the Activity Tracker page.

Injury Reported to Compensation Carrier

Indicates whether the injury was required to be reported to the public school's workers' compensation carrier.

Valid values:

  • Y - Yes

  • N - No

The field value is retrieved from the RPTD TO COMP field on the STAFF INJURY situation type of the Activity Tracker page.

Missed One or More Workdays

Indicates whether the injury caused the staff member to miss all or part of one or more work days.

Valid values:

  • Y - Yes

  • N - No

The field value is retrieved from the MISSED DAYS field on the STAFF INJURY situation type of the Activity Tracker page.

Injury Reported Publicly

Indicates whether the injury was required to be reported to the public school pursuant to the school's reporting policy.

Valid values:

  • Y - Yes

  • N - No

The field value is retrieved from the PUBLIC RPTD field on the STAFF INJURY situation type of the Activity Tracker page.

Injury Details

A description of the injury incident.

The limit is 50 characters.

The field value is retrieved from the DETAILS field on the STAFF INJURY situation type of the Activity Tracker page.
