Contribution Report - Header Record

The first record in the Contribution File must be the Header Record, which has a data length of 200. This record identifies the employer entity making the PERA contributions for a specified paid date. It also summarizes the total contributions and the number of plans included.

Employers must submit one Contribution Header Record for each group of plan summary and member detail records submitted jointly. Only one paid date can be reported per Report Header Record.

The following table outlines the Minnesota PERA retirement contribution report’s header record positions, field names, descriptions, data type, data source or valid value, and whether the field is required.
