Social Security Number | The employee's social security number. If not known, enter zeros in position 2-9 and 1 in position 10. If additional SSNs are unknown, increment the number in position 10 by 1.

State Unemployment Insurance Account Number | The state-assigned employer organization code. The MODES-4, “Missouri Quarterly Contribution and Wage Report,” sent each quarter, will have the account number printed in item 2. Do not include wages unless a 14-digit account number is assigned. FEIN and “applied for” accounts are not accepted. The account number is in this format: 345678-0-123-4567. Starting from left to right:

Earnings | The wages paid to employees are their gross before deductions, except for federally permitted cafeteria deductions. This also includes the reasonable cash value of any in-kind remuneration. Report the numerical value of this field with right justification and zero-filled using zeros only (no spaces or blanks). Probationary Code | Indicates whether the employee is on probation. If the worker was employed on a test or trial basis and was employed no longer than 28 consecutive calendar days, the value of this code should equal 1. When this code is present, there also must be a separation date. The lack of one nullifies this code.
