MI Quarterly Wage Report - Option to enter base wage and capture excess tax

Enhanced the Michigan Quarterly Wage report to enter the base wages. The base wages are then used to calculate the excess taxable earnings for reporting.

Pay Period reflecting 12/31/1899 instead of the check issue date.

Deduction Amount populating incorrectly.
Accts Payable Check No. populating the actual check no. instead of 0.
