IN Biannual Form 9 maintenance issues corrected

The following issues were discovered and corrected in the IN Biannual Form 9 record maintenance:

Updated information was not saved when the user selected the save button.
Multiple records with similar Account IDs were deleted when deleting a single record.
The Period in the selection criteria on the Download report was not printing correctly.
