AZ Quarterly Unemployment Tax Wage Report

Report Specific Set-up

Prior to using the report application the user will have to enter the UI Tax Employer Number in the Human Resources Profile – State Window (see the Human Resources Profile – State Window section under Setting Up appllication for AZ State Reporting above). In addition, a UI tax-approved branch number (or three zeros if there is no branch number) must be entered in the Human Resources Profile – State Window.

Quarterly Wage Table Maintenance

To access the AZ Quarterly Unemployment Report select the following menu path: Human Resources -> State -> AZ Quarterly Unemployment Tax Wage Report.

The AZ Quarterly Wage Report displays a list of employees and the wage information for the report.

Update the AZ Quarterly Wage Report's data

Use the fields available in the Search Criteria at the top of the report's main screen to display the employee records that you want to change, or select a line in the list, then click the OK button to get into the Employee Information section at the bottom of the main screen. Notice that you can only change two fields: Employee Wages, and Taxable Wages fields. Either of these fields will accept amounts greater than zero.

Add an employee to the AZ Quarterly Wage table

Click the New icon at the top left corner of the report's main screen.

Load Quarterly Wage Table Entries

You can load the required data for all the qualified employees, from the appropriate files, and append all employee data into the AZ quarterly wage table.

  1. Click Load.

  2. In the Load Criteria section, select the year and quarter to load.

  3. Click OK.

  4. If records already exist and you want to reload records, click Yes on the confirmation message.

Purging Quarterly Wage Table Entries

You may delete all the data records for any given quarter and year, from the AZ quarterly wage table.

  1. Click Purge.

  2. In the Purge Criteria section, select the year and quarter to delete. 

  3. Click the OK button. 

  4. To continue with the purge, click Yes on the confirmation message. 

Generating Reports and Files

  1. Click Reports.

  2. Select the report type – either 'Hard Copy' (printed report) or 'Magnetic Media' (file for submission to the state).

  3. In the Report Criteria, select the year and quarter for the report. 

  4. Click OK

  5. If you selected to output the hard copy report and you have permission to view social security numbers on reports, the following message will be displayed:

    Click Yes to print the Social Security numbers on the report, otherwise the Social Security numbers will be printed in the masked format.

  6. Select the destination for the report. The report destination page for the Magnetic Media option is displayed.

The program will extract and report information for each employee who has received the specified type of reportable wages during the reporting quarter.

  • The Hard Copy option produces a listing of the wages paid for all employees. The employees will be ordered by last name, then first name.

  • The Magnetic Media option produces a fixed length file containing each reportable employee using the record layout in the federal MMREF-1 format specified by the Arizona Department of Economic Security. The name of the file created is "qtrwage.tap". The default location for this file is the directory associated with the FASTAPDIR environmental variable. Employees with no wages will not be included in the magnetic media file.

To view the log, click the Documents icon on the main AZ Quarterly Unemployment Tax Wage Report page, and then select the file name of the log and click OK.

The hard copy report is formatted according to the AZ Quarterly Unemployment Tax Wage Report requirements.
